2010 Football and Cheer Sign-Up forms

 

 

Required forms can be downloaded below. The completed forms can be brought
to the conditioning clinics happening Saturdays at Wood School from 12:00 pm - 2:00 pm
(Beginning April 17th),
or Monday thru Friday from 6 - 8 pm at Woodstock Park (Beginning July 26th).

Players and Cheerleaders will need the following forms:

  • Current Physical (dated and stamped in 2010)
  • Equipment Deposit of $350 (Football only: check will not be cashed as long as equipment
is returned, and not damaged
  • Registration Fee (Check or Money Order only)

Announcements from the Alameda Pirate Football:

 

 

  Our fees for the upcoming 2010 season are as follows:

 

  $100 for Football

 

  $100 for Cheerleaders

 

  $75 for families with two or more children sigining up.

  so please make your registration payment with a check or money order. Read More

 

  If your child did not return all the equipment from last season

  they will not be allowed to suit up or be issued any equipment until unreturned items are paid for.

 

  San Jose PAL requires us to have a physical form (dated in 2010) from a doctor stating

  that your child is able to participate in football or cheer.

  All participants must have this form turned in by July 25, 2010

  or they will not be allowed to participate in practice.
